Community First Services and Supports is a Minnesota health care program designed to help eligible people live more independently at home and in their communities. Aavena helps you understand your choices, create a person-centered service delivery plan, and move through CFSS with informed, independent support.

EDUCATION AND ORIENTATION

We explain CFSS, your rights and responsibilities, the available service models, and the choices you can make.

SERVICE DELIVERY PLAN SUPPORT

We help you write your plan if you want assistance, review it for completeness, and submit it to your lead agency for approval.

ONGOING CONSULTATION

We provide continued education, answer questions, and support plan updates and quality conversations during your service plan year.

Two models. One important choice.

AGENCY MODEL

A CFSS provider agency serves as the employer. The agency recruits, hires, trains, supervises, and pays support workers. This may fit if you want an agency to manage most employer responsibilities.

BUDGET MODEL

You serve as the employer of your support workers and work with a Financial Management Services provider for payroll and employer-related support. This may fit if you want more direct responsibility and control.

Independent guidance—not direct care

Aavena educates, guides, and supports your planning. We do not provide hands-on CFSS care, employ your support workers, or serve as your Financial Management Services provider for the same services. This separation keeps consultation independent and focused on your choices.
