Your next steps, made simple

From assessment to an approved service delivery plan, here is what to expect when starting CFSS consultation with Aavena.

1. Complete your assessment

Your county, Tribal Nation, or managed care organization assesses your needs and determines whether you are eligible for CFSS.

2. Select Aavena

After you are found eligible, contact Aavena and let us know you would like us to be your CFSS consultation services provider.

3. Notify your lead agency

Tell your county, Tribal Nation, or managed care organization that you selected Aavena. It must authorize consultation services before covered consultation work can begin.

4. Complete intake and orientation

We learn about your goals, explain CFSS and your choices, and review important rights and responsibilities.

5. Write or review your plan

You decide how much help you want. Aavena can support you in writing the plan or review a plan you prepared.

6. Submit the plan for approval

Aavena submits the completed service delivery plan to your lead agency. Your services may begin after the plan and services are approved and your chosen provider’s intake is complete.
